About the Organization
Currently in its 16th year of service, Mobile Care Chicago (MCC) is a non-profit organization which operates mobile medical clinics called Asthma Vans. These Vans travel into low-income communities and provide no-cost care, education, and diagnosis to children with the chronic disease asthma. Through two Asthma Vans, MCC currently provides care to 1,100 patients through 3,300 patient visits yearly.
About the Position
The Community Health Worker will coordinate the transition of children with asthma coming through Roseland Community Hospital from ER dependence to on-going preventative care and disease control aboard Mobile Care’s Asthma Van. The ambition of this program is to organize both hospital care and school resources toward the best health outcomes of asthma patients, and to use available tools to create an environment in which asthma management is possible for low-income families. Additionally, this position will coordinate Asthma Van visits for a number of schools in the Roseland neighborhood, allowing the Community Health Worker to truly manage their patients’ experience from ER dependence to wellness, as well as provide on-going health education.
Major Position Responsibilities
Maintain communication points within Roseland Community Hospital
Connect Roseland ER patients with preventative care aboard the Asthma Van
Be the main point of contact for school sites in the greater Roseland area
Coordinate Asthma Van visits to defined school sites
Provide home environment inspections, individual education sessions, and case management when necessary
Help organize group visits within schools
Attend health fairs and school functions on behalf of Mobile Care Chicago
Provide engaging materials and education opportunities for children with asthma
Assist in the overall effort to organize community stakeholders around asthma care
Maintain timely reports and outcome measures
Qualifications
Excellent written and spoken communication skills
Experience either educating or organizing families around community health
Demonstrated ability to work independently
Very good logistical thinking
Friendliness and a good sense of humor
Knowledge of asthma a plus
Knowledge of the Roseland neighborhood a strong plus
Reliable access to a vehicle a must
Commercial Drivers License a strong plus
Most importantly, a commitment to social justice
